# Summer Camp Waiver, Consent and Acknowledgement

Please read the following terms carefully before completing registration.

## 1. Acknowledgement and Agreement

I confirm that I am the parent or legal guardian of the child being registered, or that I have legal authority to complete this registration on the child’s behalf.

By checking the agreement box below, I confirm that I have read, understood, and voluntarily agree to all terms of this waiver, consent, and acknowledgement on behalf of myself and my child.

## 2. Assumption of Risk and Supervision

I understand that participation in art activities, crafts, outdoor activities, walking trips, recreational activities, swimming, field trips, and other camp activities involves inherent risks.

These risks may include, but are not limited to:

* Falls, cuts, scrapes, bruises, or other physical injuries;
* Illness or allergic reactions;
* Emotional distress;
* Accidents involving art materials, equipment, playgrounds, swimming pools, transportation, public spaces, or outdoor environments;
* Serious injury or accidental harm.

I understand that some risks may be known or unknown and may occur despite reasonable safety precautions and supervision.

I acknowledge that campers participate in age-appropriate group activities under general group supervision. Staff members supervise groups of children and do not provide continuous one-on-one supervision unless a separate written arrangement has been approved by Nicole’s Art Studio.

I voluntarily accept and assume the risks associated with my child’s participation in the camp.

## 3. Medical Information, Illness and Allergies

I agree to provide complete, accurate, and current information during registration regarding my child’s:

* Medical conditions;
* Food, environmental, and medication allergies;
* Medications;
* Physical, emotional, behavioural, developmental, or accessibility needs;
* Emergency contact information;
* Any other information that may affect my child’s health, safety, or participation.

I agree to promptly notify Nicole’s Art Studio if any of this information changes.

I understand that Nicole’s Art Studio may rely on the information I provide without independently verifying it.

I agree not to send my child to camp if they are experiencing symptoms of a contagious or communicable illness, including fever, vomiting, diarrhea, or other symptoms that may place other campers or staff at risk.

If my child becomes ill, injured, or unable to safely participate during the camp day, I understand that Nicole’s Art Studio may contact me or an emergency contact and require that my child be picked up promptly.

## 4. Emergency Contact, Medical Assistance and Safety Authorization

I authorize Nicole’s Art Studio to contact me and the emergency contacts provided during registration by phone, text message, or email in an emergency or urgent situation.

If I cannot be reached, I authorize Nicole’s Art Studio staff to take reasonable and necessary action to protect my child’s health and safety.

This may include:

* Providing basic first aid;
* Contacting emergency medical services;
* Calling 911;
* Following an emergency medical plan supplied by the parent or guardian;
* Evacuating or temporarily relocating campers;
* Implementing shelter-in-place procedures;
* Taking other reasonable safety measures in response to fire, severe weather, public health concerns, facility hazards, or other emergencies.

I understand that Nicole’s Art Studio staff members are not medical professionals.

I understand that my child will remain under supervision until they are released to an authorized adult or emergency personnel.

## 5. Behaviour and Conduct

I understand that my child must follow camp rules, safety requirements, and reasonable instructions from staff and volunteers.

Nicole’s Art Studio reserves the right to require the immediate pickup, suspension, or removal of a camper whose behaviour:

* Is unsafe;
* Is seriously disruptive;
* Is threatening, violent, or inappropriate;
* Places the camper, another child, staff member, volunteer, or member of the public at risk;
* Prevents the camp from safely operating;
* Repeatedly fails to follow staff instructions or camp rules.

I understand that removal from camp for behavioural or safety reasons may occur without a refund, credit, make-up day, or prorated reimbursement.

## 6. Personal Belongings

I understand that Nicole’s Art Studio is not responsible for the loss, theft, misplacement, or damage of personal belongings brought to camp.

I agree to clearly label my child’s belongings and avoid sending valuable or unnecessary items.

## 7. Food Safety and Allergy Disclaimer

I understand that I am responsible for providing my child’s lunch, snacks, drinks, and any required food-related supplies.

Peanuts, tree nuts, and seafood are not permitted at camp.

Food sharing between campers is strictly prohibited.

I understand that Nicole’s Art Studio will take reasonable precautions regarding food allergies but cannot guarantee that the camp environment, shared facilities, public locations, transportation, or food brought by other families will be completely free from allergens or cross-contact.

If my child has an allergy or requires emergency medication, I agree to provide all required information, instructions, medication, and emergency supplies before my child attends camp.

## 8. Field Trips, Transportation and Off-Site Activities

I consent to my child participating in supervised off-site and community activities organized as part of the camp program.

These activities may include:

* Walking excursions;
* Parks and playgrounds;
* Libraries;
* Museums and galleries;
* Swimming pools;
* Community centres;
* Public events;
* Other age-appropriate community locations.

Transportation may include walking, public transit, or another method approved by Nicole’s Art Studio.

I understand that off-site activities and transportation involve risks, including traffic, falls, separation from the group, weather exposure, interactions with members of the public, and accidents in public spaces.

I voluntarily accept the risks associated with my child’s participation in off-site activities and transportation.

I understand that activities may be changed, postponed, cancelled, shortened, relocated, or moved indoors because of weather, safety concerns, facility availability, staffing, or other operational circumstances.

## 9. Swimming Activities

I understand that swimming and activities near water involve inherent risks, including serious injury and drowning.

I understand that swimming supervision is provided in a group setting and is not continuous one-on-one supervision.

I agree to accurately provide my child’s swimming ability and any water-related limitations during registration or before a swimming activity.

I understand that Nicole’s Art Studio may restrict or refuse my child’s participation in swimming activities if staff believe participation would be unsafe.

By completing registration, I consent to my child’s participation in scheduled swimming activities unless I notify Nicole’s Art Studio in writing that my child does not have permission to participate.

## 10. Sun Protection and Outdoor Conditions

I agree to provide appropriate sun protection for my child, including sunscreen, a hat, suitable clothing, and a water bottle.

I authorize staff to remind my child to apply or reapply sunscreen and to provide reasonable assistance when appropriate.

I understand that staff cannot guarantee complete sunscreen coverage or prevent all sun exposure, sunburn, insect bites, dehydration, heat discomfort, or weather-related effects.

## 11. Security Cameras

I acknowledge that security cameras may be used at Nicole’s Art Studio for safety, supervision, security, and incident-review purposes.

Camera footage will be treated as confidential and will only be accessed when reasonably necessary, required for safety or incident review, or required by law.

## 12. Authorized Pickup

I understand that my child will only be released to a parent, guardian, or another person authorized through the registration information or subsequently approved in writing.

Nicole’s Art Studio may request government-issued identification before releasing a child.

I agree to promptly notify Nicole’s Art Studio in writing of any changes to authorized pickup arrangements.

## 13. Media Permission

During registration, I will be given the opportunity to accept or decline permission for Nicole’s Art Studio to photograph or record my child.

If I provide permission, I authorize Nicole’s Art Studio to use photographs or videos of my child for purposes that may include:

* The Nicole’s Art Studio website;
* Social media;
* Camp updates;
* Promotional materials;
* Advertising;
* Printed or digital displays.

I understand that no payment or compensation will be provided for the use of approved photographs or videos.

My media permission selection will not affect my child’s ability to participate in camp.

## 14. Program Changes and Circumstances Beyond Our Control

I understand that camp schedules, instructors, rooms, activities, trips, transportation arrangements, and program details may be changed when reasonably necessary.

Nicole’s Art Studio will not be responsible for interruptions, delays, schedule changes, postponements, relocations, or cancellations caused by circumstances beyond its reasonable control, including:

* Severe weather;
* Public health emergencies;
* Government orders;
* Utility failures;
* Facility closures;
* Transportation interruptions;
* Safety concerns;
* Emergencies;
* Other circumstances beyond the reasonable control of Nicole’s Art Studio.

Any refund or credit will be determined in accordance with the Nicole’s Art Studio Refund and Cancellation Policy and applicable law.

Reasonable efforts will be made to notify parents or guardians of significant changes as soon as practicable.

## 15. Release of Liability

To the fullest extent permitted by applicable law, I release and agree to hold harmless Nicole’s Art Studio Limited and its owners, directors, employees, instructors, volunteers, contractors, and agents from claims arising from or related to my child’s participation in camp activities, including claims connected with the ordinary risks of camp activities and ordinary negligence.

This release does not apply where liability cannot legally be excluded or limited.

I acknowledge that my child’s participation is voluntary and that I accept the inherent risks associated with the camp activities described in this agreement.

## 16. Electronic Acceptance

By checking the box below and completing registration, I acknowledge that:

* I have read and understood this Summer Camp Waiver, Consent and Acknowledgement;
* I have had the opportunity to review the terms before registering;
* I voluntarily agree to these terms on behalf of myself and my child;
* My electronic acceptance has the same effect as my written signature.

### Required Agreement Checkbox

**I confirm that I have read, understood, and agree to the Summer Camp Waiver, Consent and Acknowledgement on behalf of myself and my child. I understand that checking this box constitutes my electronic signature and is required to complete registration.**

Refund and Cancellation Policy

This policy is firm and applies to all Summer Camp registrations.

Registration Confirmation

A confirmation email and/or receipt will be issued once registration and payment have been successfully completed.

The confirmation will include the camp dates and program(s) for which your child has been registered.

Parents/guardians are responsible for reviewing and retaining their registration information for future reference. If payment is made by e-transfer, please ensure you have received your invoice by email and check your Spam/Junk folder if necessary.

Refunds will not be issued due to missed camp dates, scheduling conflicts, or failure to keep track of registration details.

Registration Deposit

A $50 non-refundable registration deposit per registered week is required at the time of registration to reserve your child’s space.

The deposit is non-refundable except if Nicole’s Art Studio cancels the camp session.

Cancellation by Parent/Guardian

More than 7 days before the first day of the registered camp week

A refund will be issued minus the non-refundable $50 registration deposit per week.
Within 7 days of the first day of the registered camp week

No refund will be provided, except under the Medical Exception described below.

To request a cancellation, parents/guardians must provide:
Participant’s name
Registered camp week(s)
Reason for cancellation

No Refunds, Credits, or Make-up Days

Refunds, credits, make-up days, or prorated fees will not be provided for:
Missed camp days for any reason
Partial attendance
Late arrivals or early departures
Scheduling conflicts
Personal vacations or family plans
Inclement weather, weather-related schedule changes, or program adjustments
Requests for partial refunds after camp has begun
Credit card processing fees incurred at the time of registration
A parent/guardian choosing to withdraw a camper after camp has started

Medical Exception

The only exception to this policy is a sudden illness, injury, or medical condition that prevents the participant from attending or completing the registered camp week.

A refund may be considered only when:

A medical certificate or doctor’s note is provided;
The documentation confirms the participant is unable to attend or continue participating in camp.

If approved, any refund will be prorated based on the unused portion of the registered camp week, less the non-refundable registration deposit and any applicable processing fees.

Medical documentation must be submitted as soon as reasonably possible.

Extenuating Circumstances

Requests for refunds due to serious family emergencies or other exceptional circumstances may be considered on a case-by-case basis.
Supporting documentation may be required.
Approval of these requests is entirely at the discretion of Nicole’s Art Studio, whose decision shall be final.

Camper Withdrawal or Removal

Nicole’s Art Studio reserves the right to withdraw or dismiss a camper at any time if:
Camp safety rules are not followed;
Behaviour significantly disrupts the program;
The camper poses a safety risk to themselves or others.
In these situations, no refund or credit will be issued.

Camp Cancellation by Nicole’s Art Studio

Nicole’s Art Studio reserves the right to cancel a camp session due to:
Insufficient enrollment;
Facility issues;
Severe weather;
Public health concerns;
Staff shortages; or
Other circumstances beyond our reasonable control.
If Nicole’s Art Studio cancels a camp session before it begins, a full refund of all fees paid, including the registration deposit, will be provided.

Final Decision
Nicole’s Art Studio reserves the right to interpret and apply this Refund and Cancellation Policy.
All refund requests will be reviewed individually where applicable, and Nicole’s Art Studio’s decision regarding refunds shall be final.

I grant permission for Nicole’s Art Studio to use photographs or videos of my child participating in camp activities for promotional and marketing purposes. I understand that no personal information will be disclosed.
