Registration Form for March Break Art Camp

Waiver and Release of Liability I, the undersigned parent/guardian, hereby acknowledge and agree to the following terms and conditions for my child’s participation in Nicole’s Art Studio Art Camp:
1. **Assumption of Risk ** I understand that participation in art activities, including but not limited to painting, drawing, clay work, and outdoor activities, involves certain inherent risks. While Nicole’s Art Studio staff will take all reasonable precautions to ensure a safe environment, I acknowledge that accidents or injuries may occur. I accept full responsibility for any risk of injury or property damage that may arise from my child’s participation.
2. **Medical Emergency ** In the event of a medical emergency, I authorize Nicole’s Art Studio staff to administer first aid and/or arrange emergency medical care for my child. I understand that reasonable efforts will be made to contact me or the designated emergency contact as soon as possible. I agree to bear any costs incurred for such medical treatment.
3. **Behavior and Conduct ** I understand that my child is expected to follow the camp rules and behave respectfully towards instructors, staff, and fellow participants. In the event of serious misconduct or disruptive behavior, I understand that Nicole’s Art Studio reserves the right to dismiss my child from the camp without a refund.
4. **Personal Belongings ** I understand that Nicole’s Art Studio is not responsible for any lost, stolen, or damaged personal items brought to the camp.
5. **Release of Liability ** I hereby release and hold harmless Nicole’s Art Studio, its staff, volunteers, and affiliates from any and all liability, claims, or demands arising out of or in connection with my child’s participation in the art camp.
**Acknowledgment and Agreement ** By signing below, I confirm that I have read and understood this waiver and release of liability, and I voluntarily agree to its terms on behalf of my child.

Nicole’s Art Studio Art Camp Refund/Cancellation Policy: This Policy is Firm
Order Confirmations/Receipts are sent when your purchase is complete. (This includes exactly what you have signed up and paid for, including dates.) Please keep track of the products you have signed up for, as this is not the responsibility of Nicole’s Art Studio. If you are paying by E-transfer, please ensure you receive the invoice via email. If the invoice does not appear in your inbox, please check your Spam folder. Refunds are not granted due to losing track of your dates. (Hot Tip: Star or bookmark your confirmation email upon receiving it for future reference.)
To protect our business and operations, refunds/credits will not be provided for:
1. Cancellations within 7 days of the selected day camp.
2. Inclement weather.
3. A missed day(s) for any reason (e.g., sign up for a week and miss one or two days, or sign up for a day and do not attend, and then request a refund).
4. We do not provide prorated/partial discounts (e.g., sign up for a four-day camp, then request a refund for one day).
5. Credit card fees incurred by the business at the point of purchase will not be refunded.
Please understand that we do not track or have control over other organizations’ schedules. If you accidentally register for another camp during the same week as our art camp and there is a conflict, you will need to cancel it yourself at least 7 days before the start date. Cancellations made less than 7 days before the start date are non-refundable.
The only exception to our refund policy is in the case of a sudden illness that prevents you from attending the entire week. In this situation, please provide us with a medical certificate, and we will issue a refund.

I grant permission for Nicole’s Art Studio to use photographs or videos of my child participating in camp activities for promotional and marketing purposes. I understand that no personal information will be disclosed.
